Please complete the following:

TERMS AND CONDITIONS

  • APPLICANT shall keep and maintain the borrowed property at their own cost and expense during the term of use.
  • APPLICANT shall keep borrowed city property in a good state of repair, normal wear and tear excepted.
  • COST OF REPLACEMENT: APPLICANT shall fully reimburse the CITY for the cost of replacement of borrowed property that is not returned to the city because it is lost or stolen.
  • COST OF REPAIR: APPLICANT shall fully reimburse the CITY for the cost of repairing rented property that is damaged during the rental term. a. If damaged, APPLICANT shall be responsible for the cost of repairing borrowed property to the condition it was in at the time of pickup, normal wear and tear excepted.
  • The CITY'S invoice for replacement or repair is conclusive as to the amount APPLICANT shall pay for the repair or replacement of borrowed property.

I acknowledge by my signature that the records sought by this request will not be used for a mailing list or telephone lists and will be follow the regulations as set forth in Idaho Code §74-120.
